Multiple reports with the same field names in different datasets behave incorrectly

Multiple reports with fields with the same name shared between them clash, as the reports share the same form body.

If you have a user report and an EPrint report, and both have a date field, the date field will inconsistently not work - sometimes the date from the user part of the form will be sent off, and sometimes the date field for the EPrint part of the form.

The help button will also stop working, as it will not know which element to expand the help for and will likely expand the help for an off-screen element.
