Improve sparse binary tree child handling

Fix nil child slot handling across height/children helpers and hash/JSON serialization for sparse binary trees.

next already contains the sparse-binary child-handling fixes, especially fd4b175 and df5c6d5 .
Keeping the issue open for tracking, but the requested hardening is already in place there.
