Describe the bug:

Could not read the focused app. Must close and reopen PcPanel program after opening a new program to get it to update under 'Add app'

Steps to reproduce:

Tried many different applications nothing is found for the find focused app button.
Firefox, Steam, Palworld, WoW. Rhythmbox.

Expected behavior:

App to be found, add app drop down update as new app is opened.

It seems like you might be describing a few separate issues:

1. The app-list not populating correctly until restart: A fix was made that should address this. Note that on Linux, this will only show processes that have an active audio session
2. Focus volume not working: No real fix was made, but it should be more clear what isn't working
3. Your log also showed the media keys never finding a player in the Flatpak (No MPRIS media player found) even with music playing, the sandbox was hiding players from us. should be fixed

Did focus volume work before? For Linux, it requires either KDE Plasma (where kdotool works) or X11 (where xdotool works). When a different Wayland desktop environment is used, focus volume will not work.
echo "$XDG_CURRENT_DESKTOP / $XDG_SESSION_TYPE" should produce KDE / wayland, or an X11 alternative.

When you are on an unsupported desktop environment, the UI should show that better now. If you are on X11 or KDE wayland, let me know what the echo statement produces

The fixes should be in the v2.0.91-SNAPSHOT, the generated report from that version should also include more focus-window debug stuff (you may want to check to ensure there isn't anything private in there).
