feat: release 0.11 language assistance and reviewed workspace edits

Adds the 0.11 public language and editing APIs so external plugins can offer signature help, lazy completions, rename/format/fix proposals, and document display data through explicit host capabilities.

• Completion and action resolution bind one-use tokens to the selected provider and current document; actions also bind the accepted diagnostic bundle. Snippets and all display text use bounded data-only contracts.
• Workspace edit plans support edit/create/move/delete previews, exact reviewed digests, current authorization and per-file CAS. An optional Node intent/receipt journal records interrupted applications for read-only recovery and separately reviewed inverse proposals.
• Semantic full/delta tokens use a bounded cache and at most one full fallback within the original request budget. Folding, hints and symbol trees validate UTF-16 geometry, containment and limits; hosts validate actual result ownership before rendering.
• Includes public types, schemas, documentation and independently installed examples. Existing manifests, flat symbols, literal completions, file methods and protocol v1 remain supported. The package stays independent of private DDS implementations and product editor releases.

Validation: local npm run check passed 395 runtime tests, TypeScript, 69 generated schemas, public inventory and an independently installed 308-file package, including CLI and all four new examples. Thirteen display tests passed again after the final await-boundary check. Actual browser checks cover full/delta/fallback, folding, symbol navigation, literal HTML-like hints, provider lifecycle, desktop light/dark and mobile layout. Windows/Linux × Node 22/24 CI and the release archive compatibility/download checks are required before release.
