Release 0.1.1

Brings the documentation in line with the code, and fixes two small defects found while checking it.
The compiler, the importer's mapping, the printer and the comparison helper in tests/helpers are unchanged, so every round-trip result of 0.1.0 holds for 0.1.1.

Code

• An unexpected exception inside the importer now exits 3, like the other transforms.
  Input that can't be read as a BPMN document (XML that doesn't parse, a root that isn't bpmn:definitions, a flow whose end names nothing) raises the new MalformedDocumentError and still exits 2.
• The validator refuses a listener that has both a fenced script and a class, expression or delegate.
  The compiler wrote only the script, so the setting was lost without a word.
  A script listener's settings are now checked like any other listener's.

Documentation

• 18 decision records corrected where they disagreed with the code or cited tests that don't hold the case. ADR-0008 now says what the forked moddle descriptor keeps and changes, and ADR-0014 describes how a fork without a join prints.
• The language reference, the transform, CLI and extension READMEs, docs/bpmn-coverage.md, the golden and examples READMEs and the glossary corrected the same way.
• The extension README explains how to install a downloaded .vsix.
• packages/transform/src/operaton-moddle.LICENSE carries the MIT notice of camunda-bpmn-moddle, from which operaton-moddle.json is derived.

Checks on this tree

• build, typecheck --workspaces, lint, format:check: clean.
• npm test: 2,552 tests pass, the 205 engine tests included.
• npm run package -w packages/extension builds vscode-bpmnscript-0.1.1.vsix, and bpmns --version prints 0.1.1.
• Fuzz, seeds 1 to 2000 in document, reversed and shuffled (seed 1) order: 1,996 valid programs each, no failing seed, results identical to 0.1.0.
