## [Unreleased]

## [1.2.7] - 2026-10-12

From a heavy user's report (461 agent runs, about $632, every feature of their last release built through the pipeline):

- **New dependencies are seen where modern projects keep them.** A change to these now counts as a dependency change (medium risk, risk review) and their packages are read: poetry `group.<name>` tables, PEP 735 `[dependency-groups]`, uv `dev-dependencies`, `requirements/*.txt`, `constraints*.txt`, `Pipfile`, and the base images in a `Dockerfile` or compose file (a database upgrade can stop a service starting; a tag bump is the same dependency, the touched file is what raises the risk). The user was reviewing these by hand. **When you update:** these dependencies were invisible before, so `agent-flow audit-risk --fail-on-new` can report them as new once (a base image shows as `dependency:Dockerfile:image:node`) and a CI job using it goes red. Review the list, then `agent-flow baseline accept --all --yes`. A Dockerfile or compose edit is also a medium-risk change now, which asks for the stronger reviewer if `pipeline.models.high_reasoning` is set.
- **A push that GitHub's secret scanning blocks says why.** The escalation was cut at 300 characters, which dropped the file and the unblock URL. It now names the file, says to build a fixture's fake secret at run time, and gives the URL a person can use to allow it.
- **`agent-flow clean`** takes back the branches and worktrees a pipeline leaves behind. Every `run --pr` pushes `agent/issue-N` and nothing removed it after the merge: the user had 169 on the remote, 114 local and 83 worktrees, and most belonged to issues no longer in the state list, so `state dismiss --merged --clean` could not reach them. It previews until `--yes`, touches only `agent/issue-N`, and removes a branch only when its work is provably merged: its tip is an ancestor of the default branch, or equals the head of a merged pull request (a squash leaves no ancestor, so `gh` is asked; without it squash merges stay). A worktree with uncommitted work stays with its branch, an issue with a run or decision still open is left alone, a pipeline role may not run it, and `--offline` leaves the remote untouched. Remote branches go in batches through `git push origin --delete`. `status` mentions it once 15 or more local `agent/issue-N` branches have piled up, and not before.
- **`state dismiss --merged --clean`** also removes the worktree and local branch of every issue it drops (never one with uncommitted work, and a branch only when git says it is fully merged), plus a branch left after its worktree was removed. The user had 83 worktrees and 114 local branches, and cleaned 166 remote branches by hand.
- **The orchestrator skill says how long a run takes** (one to two hours) and that a shell tool's background default of 30 minutes kills it mid-run; `agent-flow run <issue>` resumes. The README FAQ answers this, the leftover-branch pile and the fixture-secret push block.
- **The reviewer is asked to attack what it reviews:** for a check, gate, verifier or signer in the diff, find a way to skip it, forge its input or reorder its steps; for erasure or migration code, what a retry, a partial failure or an existing install does; for an upgrade or new base image, whether the service still starts on an existing install. Those are the kinds of bug this user's own deep review later caught. Whether it catches them is not measured.
- Already fixed in 1.2.6 and reported here: finished issues merged by hand no longer stay "ready" (77 for this user), and guard false blocks from Git Bash paths.

- **Branding.** The README now opens with one sentence and one outcome (*Leave your coding agent alone with your repo. Come back to a reviewed pull request, not a mess.*), shows real `status` output, gives one install command and two follow-up steps, and puts the other install routes, the CI snippet and the command list under collapsed sections. A short "From one real repo" block gives measured numbers from five days of use, including the false alarms. The same sentence is the description in `package.json`, the Claude and Codex plugin manifests, the marketplace file, `gemini-extension.json` and the GitHub About text. No behaviour changed.
- **Enrolled in Anthropic's OSS Scanner.** `.oss-scanner/Dockerfile` builds and tests the project the way CI does, and `.oss-scanner/threat_model.md` tells the scanner to treat the agent as the attacker, which components matter, how we rate severity and which gaps `SECURITY.md` already admits. Neither ships in the npm package. No behaviour changed.

**What if my harness has no hooks?** The skills and the pre-commit gate still apply; the guard's per-call blocking does not. The harness matrix says which is which.

**My run stopped after 30 minutes.** That is the background limit of the tool that launched it (Claude Code's default), not Agent Flow's: a pipeline takes one to two hours. Launch it with the tool's longest limit, and `agent-flow run <issue>` resumes a stopped one. `--timeout <seconds>` sets how long each role may run (default 3600).

**Branches and worktrees pile up.** Every `run --pr` pushes `agent/issue-N`, and nothing removes it after the merge: one user had 169 on the remote. `agent-flow clean` previews and `agent-flow clean --yes` removes the ones whose work is already in the default branch, local and on `origin`, with their worktrees. It never touches uncommitted work, an issue with a run or decision still open, or a branch that isn't named `agent/issue-N`; a squash merge is recognised through `gh`. To stop the remote ones piling up at the source, turn on "Automatically delete head branches" in the repository's GitHub settings.

**GitHub blocked my push over a secret in a test.** Its push protection scans every commit and doesn't know a fixture from a leak. Build the fake value at run time (split or join the string) so nothing secret-shaped is committed. The run says which file it found and where a person can allow it.

**How do I remove it?** `agent-flow uninstall` previews, `--yes` applies. Files you edited are kept.
