Codex preparation failures currently identify neither the failed initialization step nor whether the owner context ended. Record one failure-only log with the static phase, existing Session ID, elapsed preparation time and owner context status. Native error text, configuration and credentials stay out of this log; successful preparation adds no log, and execution, cancellation and cleanup behavior stay unchanged.
Validation: focused race tests cover success, rejected MCP configuration, cancellation and cleanup; Go vet and repository name, documentation and CI checks pass. Coordinator reviewed the complete diff.
