Why

Prod forensics (14 days, 2026-08-19): extraction is 88% of BYOK input tokens, and input is ~92% of all BYOK tokens. A live retry storm was the biggest single cause — the owner's Groq model started returning 404 around 08-17, and every extraction sweep since has re-attempted the same ~35 messages (failure_categories: {'http_404': 35}, every row stuck at attempts=1, the cap never binding).

Root cause (retry storm)

claim_action_item's INSERT ... ON CONFLICT DO NOTHING never set execution_options(preserve_rowcount=True). psycopg3 reports rowcount == -1 for that statement, which is truthy, so losing the conflict still returned a phantom claim token. The real reclaim path — the one that bumps attempts — never ran, and the "skipped" bucket those attempts surfaced under bypassed the existing failed-bucket breaker. One line fixes the phantom; the rest of this PR makes the cost of a dead credential bounded even when something else goes wrong.

What changed (apps/api only)

Retry-storm class (P0)

  • claim_action_item: preserve_rowcount=True on the insert (the actual fix).
  • record_extraction: credential-class failures (http_401/403/404) jump attempts to MAX_ATTEMPTS, guarded by an atomic active-credential identity check under the per-user app_user lock. If a credential rotation raced the attempt, the row settles failed, attempts=0 instead so the new credential gets a clean shot.
  • llm_settings routes: reset attempts=0 on the user's failed rows only when the ACTIVE credential actually changes (material PUT, activate switch, first create that becomes active). Creating an inactive spare, flag-only PUTs, and the kill switch do not reset.
  • Sweep breaker reads the per-attempt failure category instead of the bucket (so skipped-but-credential-dead attempts count), and the task result gains an additive aborted count.

Force re-extraction (P1)

  • force=True only steals settled extracted/no_action rows whose model_version IS DISTINCT FROM the current provider:model; failed rows stay claimable; pending rows are never stolen. no_action rows now stamp model_version too, so the comparison works for them.

Prompt and cap trims (P2) — token counts are tiktoken cl100k approximations; the ratio is the claim, not the absolute number.

  • Extraction prompt ~367 → ~279 tokens. Every rule and JSON key kept. received_at moves out of the prompt and into the message text as a Received: line, so the instruction block is byte-identical across calls.
  • Classification prompt ~505 → ~369 tokens (7 examples → 3, all label definitions and boundary rules intact).
  • Classification content cap 6,000 → 3,000 chars, as a named constant with a boundary test. Prod p50 body is ~2,450 chars, so the median call is untouched. FEEDBACK_TEXT_MAX stays 6,000.

Tests

  • New live-PG tests/test_extraction_concurrency_integration.py (added to the CI integration invocation): a reclaim of an existing failed row is never silently dropped; two concurrent claims on a brand-new row self-limit; a stale-lease competing reclaim is bounded by the cap-jump.
  • Full D2 reset matrix in test_llm_settings.py; category-aware breaker, aborted, and model_version-gated force in test_extraction_run.py; prompt-location and 2,999/3,000/3,001 boundary tests.

Deploy notes

  • No migrations. No env changes. No web changes.
  • The prod credential is still 404ing (checked 2026-08-20 18:01 UTC). After this deploys, each sweep spends at most ~3 wire calls before the breaker trips and the remaining failed rows hit the cap. Updating the Groq model name in AI settings resets those rows and brings extraction back.

Summary by CodeRabbit

  • New Features
    • Improved extraction recovery when credentials change or are replaced.
    • Added model-version awareness for forced reprocessing.
    • Added clearer failure handling, retry limits, and automatic stopping after repeated credential failures.
    • Extraction now uses message receipt times to interpret relative deadlines.
    • Improved classification guidance and limited classification input to 3,000 characters.
  • Bug Fixes
    • Prevented concurrent extraction claims from processing the same item.
    • Improved stale-claim recovery and credential-specific retry behavior.
  • Tests
    • Expanded coverage for extraction concurrency, retries, recovery, and classification limits.

The extraction sweep kept re-attempting the same ~35 messages every
run against a dead Groq model, with attempts frozen at 1. Root cause:
claim_action_item's INSERT ... ON CONFLICT DO NOTHING never set
preserve_rowcount, and psycopg3 reports rowcount=-1 (truthy) for it,
so losing a conflict still returned a phantom claim token. The real
reclaim path that bumps attempts never ran, and the "skipped" bucket
those attempts surfaced under sailed past the failed-bucket breaker.

- claim_action_item: preserve_rowcount=True on the insert
- record_extraction: credential-class failures (http_401/403/404)
jump attempts to MAX_ATTEMPTS, guarded by an atomic active-credential
identity check under the per-user app_user lock; a rotation that
raced the attempt settles failed/attempts=0 instead
- llm_settings: reset attempts=0 on failed rows only when the ACTIVE
credential actually changes (material PUT, activate switch,
  first-create-active)
- sweep breaker reads the per-attempt failure category instead of the
bucket, reports how many candidates it aborted
- force=True only steals settled rows whose model_version IS DISTINCT
FROM the current provider:model; failed rows stay claimable, pending
never stolen; no_action rows now stamp model_version too
- new live-PG concurrency tests pin the reclaim/phantom-token behavior
Input tokens are ~92% of BYOK spend and these two prompts are sent on
every call, so shorter instruction blocks are the cheapest win left
after the retry-storm fix.

- extraction prompt ~367 -> ~279 tokens (cl100k, approximate); every
rule and JSON key kept. received_at moves out of the prompt and into
the message text as a Received: line, so the instruction block is
the same bytes on every call
- classification prompt ~505 -> ~369 tokens: 7 examples -> 3, tighter
wording, all label definitions and boundary rules intact
- classification content cap 6000 -> 3000 chars as a named constant
with a boundary test; prod p50 body is ~2,450 chars so the median
call is untouched. FEEDBACK_TEXT_MAX stays 6000 (comment corrected)

📝 Walkthrough

Walkthrough

Extraction now tracks credential and model context across retries, resets failed attempts after active credential changes, changes the sweep breaker to use credential failure categories, stabilizes extraction prompts, limits classification input, and adds real-Postgres concurrency coverage.

Changes

Extraction reliability

Layer / File(s) Summary
Claim and credential persistence
apps/api/app/services/nlp/persistence.py, apps/api/app/services/nlp/providers.py, apps/api/tests/test_extraction_run.py
Claims and extraction results now carry model and credential metadata. Forced claims use model-version checks. Credential-class failures cap or reset attempts after an atomic identity check.
Extraction claim and sweep orchestration
apps/api/app/services/nlp/extraction_run.py, apps/api/tests/test_extraction_run.py
Extraction propagates failure categories and credential metadata. Sweeps track consecutive credential-class failures and report aborted candidates.
Credential mutation recovery
apps/api/app/routes/llm_settings.py, apps/api/tests/test_llm_settings.py, apps/api/app/db/models/llm_credential.py
Material credential changes and active-credential switches reset failed action-item attempts. Flag-only edits, inactive creates, no-op activation, and deletes do not reset them.
Classification and extraction input construction
apps/api/app/services/nlp/classifier.py, apps/api/app/services/nlp/extractor.py, apps/api/tests/test_classifier.py, apps/api/tests/test_extractor.py
Classification input is capped at 3,000 characters. Extraction uses a static prompt and places received_at in message content.
Postgres concurrency validation
apps/api/tests/test_extraction_concurrency_integration.py, .github/workflows/ci.yml, apps/api/tests/test_actions_integration.py
Real-Postgres tests cover retries, concurrent claims, stale leases, and failure caps. CI runs the new integration module.

Estimated code review effort: 4 (Complex) | ~60 minutes

Merge Risk: 🟡 Moderate · up to dbb67

The PR changes retry and credential-reset behavior to bound extraction spend, but the current head can still reset failed attempts for an unchanged API key, allowing repeated failures to bypass the cap; related tests also have setup issues that can miscount claims or depend on live PostgreSQL. Merge readiness is moderate until these bounded correctness and verification risks are fixed or explicitly accepted.

Compare the API key value before resetting attempts.

At Line 767, api_key_provided is true when the request repeats the stored API key. The active credential then remains unchanged, but Line 792 resets every failed row. This reopens rows capped for the same failed credential.

Reset only if target.api_key != effective_api_key. Add a regression test that submits row.api_key with unchanged provider, base URL, and model, then asserts db.reset_calls == [].

Two _claim_extract_record stand-ins still return a 2-tuple.

This file updates most monkeypatched stand-ins to the new (bucket, user_id, category) contract, but two remain two-element: the fake_claim at Line 1939 and the one at Line 2023 both return "extracted", uuid4().

run_extraction_sweep unpacks three values inside its try block, so those calls raise ValueError, the generic except Exception catches it, and each candidate is counted as failed. The affected tests no longer assert the behavior they describe.

Update both stand-ins to return "extracted", uuid4(), None.

Run this script to list every remaining 2-tuple stand-in:

Record worker exceptions so a real failure is visible.

If a worker raises (for example BrokenBarrierError on a timeout, or a driver error), results stays incomplete and the test fails with KeyError: 'A'. The original exception is lost. The join(timeout=...) result is also unchecked, so a hung thread reports the same way.

Capture per-thread exceptions and assert thread completion before reading results.

Re-sending the exact stored key counted as a material change, so it
reset every capped row and cleared last_verified_at; and revision
bumped on every PUT, so a flag-only edit racing an in-flight 404
tripped D2's identity fence and reopened that row. Compare the key
value, and bump revision only when credential contents change.

Also restores 'click' in classification boundary rule 2 (dropped by
the trim), fixes two test stand-ins still returning the old 2-tuple
(they were passing via the containment path), and surfaces worker
exceptions in the concurrency tests.

Addressed in 9e339db:

  • llm_settings.py same-key PUT / revision: material_changed now requires effective_api_key != target.api_key (EncryptedText reads back plaintext), and revision += 1 moved inside the material branch — a flag-only edit or identical re-send no longer resets attempts, clears last_verified_at, or bumps revision. Regression test test_put_resend_of_the_identical_stored_key_does_not_reset_extraction_attempts added; the flag-only revision test inverted.
  • test_extraction_run.py 2-tuple stand-ins: both now return ("extracted", uuid4(), None) and assert result["extracted"]/result["failed"]. Grep confirms no other 2-tuple stand-in remains.
  • concurrency test diagnosability: worker exceptions captured and asserted before reading results, is_alive() checked after join.

Move the extraction-attempt reset into persistence.

_reset_extraction_attempts owns a reusable ActionItem database update in the route module. Move it to apps/api/app/services/nlp/persistence.py and call it through persistence. This keeps the route handler thin and centralizes extraction persistence behavior.

As per coding guidelines: “Keep route handlers thin and place reusable behavior in services.”

Include provider_account_id on the seeded MailMessage.

Keep apps/api/tests tests offline.

This fixture creates a live PostgreSQL engine, installs the vector extension, creates tables, and truncates app_user. The file matches apps/api/tests/**/*.py, where tests must remain deterministic and offline.

The failed-row attempts reset is extraction persistence behavior, not
route logic, so it lives next to the cap-jump it undoes. Also drops an
unused seed parameter in the concurrency test.

Round 2 addressed in the latest commit:

  • Move the reset into persistence — done: persistence.reset_failed_extraction_attempts, called from the four route sites; route-local copy removed.
  • _seed ignores model_version — the parameter was unused (no caller passed it and no action_item row is seeded); removed it rather than wire it to Classification.model_version, which is the classifier's version and unrelated to the extraction model_version the tests care about.

Declined, with reasoning:

  • provider_account_id on the seeded MailMessage — MailMessage has no provider_account_id column; account identity lives on MailThread (which the seed does set). The guideline applies to thread/sync-run writes here.
  • "Keep apps/api/tests offline" — this file follows the repo's existing opt-in integration tier (test_auth_integration.py, test_feedback_integration.py, test_actions_integration.py, test_llm_credentials_integration.py, test_seed_demo_integration.py): it auto-skips without TEST_DATABASE_URL, refuses any DB not ending in _test, and CI runs it only in the dedicated DB job. The default pytest run stays deterministic and offline (1356 passed, 37 skipped locally).
