Add App Health endpoint telemetry

Summary

• Add privacy-bounded App Health endpoint telemetry with the official Hono/Worker adapter.
• Use Hono route templates, the existing optional ingest key, Worker waitUntil delivery, and release/environment metadata.
• Pin App Health v0.4.0 from its official tarball with lockfile integrity.
• Add tests for dynamic route normalization, excluded request values, disabled collection, and nonblocking delivery.
