Adds AuthKit TOTP enrollment, verified-factor listing, challenges, verification, and deletion. Pending enrollments stay out of the active list, and each challenge can succeed only once. Setup keys, QR codes, URIs, and one-time codes are redacted from the request ledger.
Validated with the official WorkOS SDK: 18 WorkOS tests, core tests, type check, and package build pass. The MFA lifecycle matches a check against WorkOS's test environment.
