fix(pre-open): stamp artifact generator provenance

Summary

Stamp full and compact pre-open gate artifacts with the github-delivery package version plus SHA-256 digests of the gate and review-scope implementations that produced them.

Trace evidence

The newest Grok run inherited an old preopen-full.json where large_diff still implied incomplete review. Current source had already changed that semantic. Because the artifact carried no generator provenance, Grok had to re-read implementation files and reverse-engineer why the saved JSON disagreed with current behavior.

Fix

Every newly emitted pre-open artifact includes:

Compact summaries preserve the same identity.

This does not trust old artifacts more; it makes stale/unknown-generation artifacts immediately recognizable.
