Adds an outcome rail beside the first decision. Agent scenarios separate exact-rule matching, host approval, and tool invocation; interface scenarios show the observed browser gate event and element state. A one-click counterexample flips the destination or declared threshold, and the inline build link follows the selected boundary. Keeps an unavailable gate or mismatched vector from suggesting a successful result.
