MEDIUM counts brought back in line with `records/`. #294 moved the record
from 6.1 MEDIUM to 8.1 HIGH, and the README kept the old values. New
`tests/test_readme_sync.py` checks the record index, the Stats block, the
records badge and the index summary against `records/` (draft records,
which need not carry a score yet, are left out), with a fixture test for
each refusal it can make. `CONTRIBUTING.md` now says when a record change
needs a README update.
- New standing check, `scripts/check_mutation_thm_trail.py`: soft-warns
on records where `mutation_count` is 1 or higher but `aivss.notes`
has no discoverable mention of the ThM decision made when that
