chore(events): remove the pre-#283 subscription upgrade code - #284

Every subscription made before #283 has been refreshed onto the new storage, and prod KV holds no mcp-events:ticket keys. Drops the version-1 policy state parser, the legacy KV ticket cleanup and its test, and fixes docs that still described the ticket and ingress HMAC.

Removes the code that upgraded MCP Events subscriptions created before #283: the version-1 policy state parser, the legacy
mcp-events:ticket:cleanup and its test.Every pre-#283 subscription has since refreshed onto the new storage, and prod KV holds no
mcp-events:ticket:keys (checked 2026-10-13 17:39 UTC). Also fixes two doc lines that still described the old ticket and ingress HMAC.npm run checkran.
