Bumps to 1.4.2 and cuts the CHANGELOG section for PRs #50 and #51:

• Blocking semantic no longer fails runs whose rows all stay within min_similarity.
• A severity-only fail carries a reason naming each comparison; the recommendation names the real cause.
• Tool evaluators skip pairs offered no tools instead of booking them unmeasured.
• The run-insights narrative only blames budgets that actually broke.

Docs audit: DOCS.md, llms-full.txt, docs/methodology.md, docs/configuration.md, docs/agents.md and docs/faq.md were updated in #50/#51. This PR also fills the one remaining gap, docs/evaluators.md (semantic tolerance in the paired test; inherited tool evaluators skip tool-free pairs).

make ci green locally (3093 passed, 95.00% coverage). After merge, tag the merge commit v1.4.2 to publish.
