Clarify review commit-readiness objective and blocker substantiation - #154
Define review as a commit-readiness assessment rather than a requirement to produce feedback. Allow approval with zero findings after required review and checks are complete.
Require blockers to identify an applicable problem, supporting evidence that accounts for existing safeguards, and why the problem must be fixed before commit. Discard unsupported or already-handled claims; retain optional concerns only when they have concrete value.
Add evidence and commit-readiness impact to the shared finding format and review template. Preserve substantive design, maintainability, and testing review.
Align initial and continuing review paths so optional suggestions alone do not force another unattended round. Guard approval and verdict correction with completed evaluation, passing applicable mandatory gates, and absence of substantiated blockers; preserve attended opt-in for optional improvements.
Bump the shared protocol and code-review skill versions, including required dependent-skill patch bumps.
Version and dependency-cascade validation: passed.
Skillsaw v0.11.4: 0 errors.
Offline link checking: passed, 0 errors.
Scoped Markdown lint: all 10 changed files checked, 0 issues.
Full-diff self-review and an 11-scenario static approval-path matrix, including incomplete/failed gates and contradictory approval with a blocker. This verifies instruction consistency, not empirical agreement between reviewers.
Existing baseline issue
Repository-wide Markdown lint still reports MD059 in skills/gh-stack/references/commands.md. The same failure was reproduced on unchanged main; this PR does not modify that file. Changed-file Markdown lint passes.
The shared _shared/review-protocol.md defines code review as a commit-readiness assessment. Approval requires complete evaluation, passing applicable mandatory gates, and no substantiated blockers. Zero findings is a valid approval outcome. Findings must include evidence and commit-readiness impact. A blocker must identify a concrete problem, account for safeguards, and explain why it must be fixed before commit. Formal verdict reports must include criteria coverage.
The code-review package aligns its start and continue workflows with the protocol. Incomplete evaluation or unmet mandatory gates keep a review open. When readiness checks pass, nonblocking findings alone do not trigger another unattended review round. The finding template adds a criteria-coverage table, evidence guidance, commit-readiness impact, and Performance as a category.
The shared _shared/recipes/self-review-gate.md clarifies that PASS means no remaining substantiated blockers. It does not certify mandatory checks or branch readiness. Callers must confirm that applicable checks pass and validation covers the final diff.
The bugfix package updates its PR workflow to track candidate snapshots across review and validation. It repeats validation when the candidate changes, re-enters review when needed, and stops with a FLAG when changes remain after the review-round budget is exhausted.
The bugfix, cve-fix, e2e, implement, and ui-implement packages receive patch-version updates. The code-review package receives a minor-version update. These changes update shared review guidance and establish cross-package conventions for approval, evidence, mandatory checks, and validation of the final candidate. Test and validation results are not established by the supplied change summary.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
Review guidance now requires complete criteria coverage, evidence-based blocker assessments, and passing applicable readiness gates. The self-review recipe distinguishes its PASS result from branch readiness. The bugfix workflow compares reviewed and validated candidate snapshots and repeats validation or review when they differ.

Summary
Review criteria and finding contract _shared/review-protocol.md, code-review/templates/code-review.md
The protocol defines criteria coverage, finding fields, blocker substantiation, and verdict rules. The template adds criteria coverage and fields for evidence and commit-readiness impact.
Review execution and supporting guidance code-review/skills/start.md, code-review/skills/continue.md, _shared/recipes/self-review-gate.md, */SKILL.md
The review instructions apply coverage and readiness rules to attended and unattended reviews. The self-review recipe distinguishes PASS from branch readiness. Six skill versions and the recipe version change.
Bugfix candidate validation
Layer / File(s)
Summary
Candidate snapshots and review convergence bugfix/skills/pr.md
The workflow snapshots candidate paths, statuses, and content digests. It reruns validation or review when the reviewed candidate differs from the validated candidate.
The updated workflows can approve or publish changes without complete readiness evidence, and can miss certain candidate changes entirely. These gaps should be fixed before merge.

Full details: Title check
Explanation
The title "Clarify review commit-readiness objective and blocker substantiation" matches the main change. The PR defines review as a commit-readiness assessment, requires blockers to show applicable problems, evidence, and before-commit necessity, and updates the shared protocol, finding format, and review template. The title is specific and concise, and it names the primary change. It is 68 characters and 7 words, which is within a reasonable length for a title.
Full details: No-Absolute-Paths-In-Skills
Explanation
PASS. The changed workflow Markdown files add no hardcoded absolute filesystem paths. Added paths are relative, such as ../../_shared/... and .artifacts/...; other slash-prefixed text is command syntax or prose. No /home/, /Users/, /tmp/, /var/, or /opt/ path was introduced.

Explanation
All six changed SKILL.md files are under 30 total lines at the PR head: bugfix 26, code-review 23, cve-fix 24, e2e 26, implement 25, and ui-implement 27. The changed-file inventory contains no other SKILL.md files.
Full details: Command-Colon-Notation
Explanation
PASS. The pull request changes no files under */commands/*.md. The repository contains 92 matching command files, and each has frontmatter name in {parent-workflow}:{phase} form with a matching workflow prefix, including examples such as bugfix:assess, design:ingest, and ui-implement:validate.
Full details: No-Orphaned-References
Explanation
No dangling or orphaned references were introduced. The changed concrete Markdown paths resolve, including ../../_shared/recipes/validation-gate.md, ../../_shared/recipes/self-review-gate.md, ../../_shared/review-protocol.md, workflow guidelines, controllers, and code-review templates. Root-level AGENTS.md, CLAUDE.md, and CONTRIBUTING.md references also exist. All skill and command files in the six touched workflows remain reachable; base-to-head reference counts did not change.
Full details: No-Content-Duplication
Explanation
PASS — The changed architectural files are six workflow SKILL.md files. The authoritative PR diff shows one version-line change in each and no added instruction block or paragraph. Therefore, the PR introduces no multi-line content that can duplicate guidelines.md, controller.md, dispatch.md, or completion.md.
Full details: Step-Sequencing
Explanation
No step-sequencing failure was introduced. The changed workflow files are bugfix/skills/pr.md, code-review/skills/continue.md, and code-review/skills/start.md. Their main headings remain sequential within each workflow: continue.md uses Steps 1–10, and start.md uses Steps 1–9. bugfix/skills/pr.md retains its pre-existing Step 0–10 and Step 3a structure; the pull request only expands the content inside Step 6 and adds a nested review-loop list, not new main steps or renamed step headings.

Review feedback fixes for PR flightctl#154:
1. Self-review gate PASS contract (self-review-gate.md v0.1.0→v0.1.1):
- Scope PASS to criteria evaluation with no substantiated blockers,
not unconditional commit/publish approval
- Require discovered-tool path (Step 1) to cover all protocol criteria
or complete remaining via Steps 2-5 before reporting PASS
- Require boundary callers to ensure mandatory checks pass and
validation is current for the final diff after gate fixes
- Add revalidation requirement to bugfix/pr.md Step 6 when gate
makes code fixes (cve-fix and implement callers already correct)
2. Severity table: align "before merge" → "before commit" in both
CRITICAL and HIGH rows to match commit-readiness boundary
3. Evaluation completeness: add "or the required evaluation is not
complete" to all three unattended local commit-readiness checks
(start.md:378, continue.md:446, continue.md:515)
4. Template categories: align to protocol canonical names
(Error handling, Naming and clarity, Test coverage, Project conventions)

Constrain commit-readiness impact by severity in the template.
The protocol defines the mapping, and the workflow approval branches require nonblocking findings to be MEDIUM/LOW. A HIGH finding marked nonblocking therefore does not reach approval through the documented paths. The template still permits this inconsistent pair and should make the constraint explicit.
Proposed clarification
-- **Commit-readiness impact:** {blocking | nonblocking}+- **Commit-readiness impact:** {blocking for substantiated CRITICAL/HIGH | nonblocking for MEDIUM/LOW}
This is a minor template-clarity issue, not a major data-integrity issue.

The template retains the exact blocking | nonblocking values and now includes the existing protocol mapping locally: substantiated CRITICAL/HIGH findings are blocking; MEDIUM/LOW findings are nonblocking. This is a template clarification, not a new policy or approval gate.

Apply the commit-readiness check before accepting done.
The earlier Step 7a check covers criteria coverage. It does not check mandatory gates or substantiated blockers. In the attended branch, done proceeds directly to cleanup. An unrun mandatory gate or substantiated blocker can therefore reach completion.
Apply the protocol's three-condition check before compiling the summary. If any condition fails, keep the review incomplete and do not proceed to Step 10.

Major · Include file mode and type in candidate identity. · pr.md:404-412
A tracked executable-bit change or regular-file/symlink replacement can keep the same path, status, and content digest. The snapshots can then compare equal even though the candidate changed.
The workflow can therefore skip validation after a self-review fix or skip self-review after validation changes the file. Include file mode and file type in snapshot equality.

Stage only snapshot-listed paths. Do not force-add an ignored file that the snapshot omits.
The candidate snapshot cannot see ignored untracked files. Step 7 then stages by explicit path, outside the reviewed and validated set. An ignored source file that the author force-adds can therefore reach the commit without review or validation. This conflicts with the Step 6 rule against committing an unreviewed or unvalidated candidate.
Suggested fix

If a needed source file is git-ignored and absent from the snapshot, stop, add it to the snapshot, and re-run Steps 5 and 6 before staging.

1. Apply the commit-readiness check before accepting done
code-review/skills/continue.md now applies the shared three-condition check before all verdict routing, not only within CHANGES_REQUESTED. The attended done path rechecks it before compiling the summary or cleaning up. Incomplete evaluation, unmet mandatory gates, or substantiated blockers cannot be waived by an APPROVED label or a done response. Optional suggestions and clean zero-finding approval remain supported.
2. Revalidate after committing self-review fixes
The UI publisher now requires actual /validate execution after a gate-fix commit, an exact PASS from the existing ## Result section, and ## Validated HEAD matching the current local HEAD. It checks freshness again before pushing; it does not merely replace the SHA in an old report or require a legitimate remote ancestor to equal a new unpublished local commit.
The related implementation publisher also reruns its required profile checks on the post-fix candidate before committing. The E2E publisher already does this and was left unchanged. No-fix paths avoid redundant validation, and failures or further unreviewed changes stop or return to the existing validation/review workflow rather than creating an unbounded cycle.
3. Include file mode and type in candidate identity
Candidate identity now includes Git-relevant mode, kind, and a consistent kind-correct payload identity across tracked/untracked/staged states. Symlinks use their link-target text, not dereferenced contents, so executable-bit changes, regular-file/symlink replacements, and retargeting are not collapsed into an unchanged content digest. Unsupported kinds are reported as unhandled; timestamps, ownership, and non-Git permission bits are excluded to avoid false churn.
4. Stage only snapshot-listed paths
The commit boundary now compares the actual staged entries—including kind, mode, and blob identity—with the converged reviewed-and-validated candidate. Extra, missing, or changed entries stop the workflow; it does not automatically reset or unstage unrelated user work.
A required ignored file must first be explicitly included as read-only candidate input and reviewed/validated before authorized staging. The previous instructions did not themselves specify force-add, so this is a narrow strengthening of the candidate invariant, not a claim that force-adding unseen files was instructed.
Validation
Changed-file Markdown, structural/version, Skillsaw, and offline-link checks pass. Disposable Git reference checks exercised frozen-candidate/index matching, rejection, and preservation; the workflow routing was reviewed statically. These checks verify the documented rules, not empirical agreement between AI reviewers.
Amend the shared review protocol to define the review objective as
judging commit readiness, not finding something to comment on. Zero
findings with APPROVED is a valid outcome when required evaluation and
mandatory quality gates are complete.
Key changes to _shared/review-protocol.md (v0.2.0 → v0.3.0):
- Add Review Objective and Stopping Rule section: no minimum findings
quota, mandatory gates still apply, optional suggestions nonblocking
- Add Evidence and Commit-readiness impact fields to finding format
- Add Blocker Substantiation section: concrete problem, code evidence
accounting for existing safeguards, necessity before commit
- Add Verdict Rules with commit-readiness check: APPROVED requires
complete evaluation, all mandatory gates passing (not unrun/incomplete),
and no substantiated blockers; CHANGES_REQUESTED for unmet gates or
substantiated blockers
- Strengthen Validation Rules: verify applicability against existing
guards; downgrade unsubstantiated blockers only if a validated useful
concern remains, otherwise discard
Align code-review workflow consumers (v0.4.4 → v0.5.0):
- templates/code-review.md: add Evidence, Commit-readiness impact, and
Design category to canonical finding format
- skills/start.md: commit-readiness check on all approval paths; prevent
unattended auto-loop on purely nonblocking suggestions
- skills/continue.md: commit-readiness check guards every verdict route
including override logic; unmet/incomplete gates prevent APPROVED even
with no code-level blockers; nonblocking suggestions alone cannot
force another round
Transitive cascade bumps (review-protocol → self-review-gate consumers):
bugfix 0.9.5, cve-fix 0.5.1, e2e 0.8.5, implement 0.13.1,
ui-implement 0.2.1

… completeness, template categories

…, prose
1. Severity table (protocol:138): HIGH action "Should fix" → "Must fix
before commit" to match existing blocker semantics (CRITICAL/HIGH
are blocking per verdict rules and commit-readiness impact field)
2. Review-validation convergence (bugfix/pr.md Step 6): final candidate
must be both reviewed and validated with no intervening code changes.
Track whether validation remediation changed candidate code; re-enter
self-review consuming remaining MAX_ROUNDS budget; stop with FLAG if
unable to converge. Candidate-code detection uses git diff on
working-tree files, excludes artifact/log churn.
3. Criteria coverage record (protocol + template + start/continue):
- Protocol: define Criteria Coverage section — evaluated, not
applicable (justified), or incomplete for every criterion;
completeness requires all applicable evaluated, unjustified N/A
or incomplete cannot support APPROVED
- Template: add Criteria Coverage table between Summary and Findings
- start.md/continue.md producer: instruct reviewer to fill coverage
table for every criterion
- start.md/continue.md consumer (Step 7/7a): check coverage table
before proceeding to verdict; do not invent omitted coverage
4. Prose consistency: "not done" → "not complete" in three unattended
local branches (start.md:384, continue.md:455, continue.md:525)

Address coordinator final-review concerns A/B/C:
A. bugfix/pr.md convergence: record explicit reviewed-candidate
snapshot (tracked staged+unstaged AND relevant untracked files,
excluding .artifacts/ churn) before revalidation. Compare
post-validation candidate against snapshot to detect actual code
changes including new untracked files. Re-review only on genuine
candidate content change, not on unchanged successful validation.
B. bugfix/pr.md round budget: initialize convergence budget=3 once,
pass remaining budget as MAX_ROUNDS on each re-entry, consume
actual reported rounds. A candidate that consumed the last round
is valid if both gates passed on it; FLAG only when re-review is
needed with no budget remaining.
C. review-protocol.md Criteria Coverage: scope the coverage-record
requirement to structured formal review reports with a verdict
(APPROVED/CHANGES_REQUESTED), not the self-review gate's compact
PASS/FLAG summary which uses a different output format.

Replace path-list-based candidate comparison with content-digest
snapshot (tracked staged+unstaged + relevant untracked files, each
with path/status/content-hash). Detects content changes to existing
untracked files, new files, deletions, and renames — not just path
list differences. Excludes only .artifacts/ outputs.
Move budget initialization before first gate invocation. Pass
review_rounds_remaining as MAX_ROUNDS in the parameter block (not
hardcoded 3). Deduct actual reported Rounds:{N} after each gate
return. Allow last-round PASS when candidate is converged; FLAG only
when re-review is needed with zero remaining.
State machine: snapshot after Step 5 as validated_candidate, snapshot
after gate PASS as reviewed_candidate, compare digests for equality,
re-enter review/validation only on actual content divergence.

1. bugfix/pr.md Step 6 item 7: make re-entry target explicit as
"Review loop, item 1" rather than ambiguous "step 1"; clarify
that initialization block and budget are not re-run/reset.
2. code-review template: add compact severity-to-impact mapping hint
(CRITICAL/HIGH substantiated → blocking; MEDIUM/LOW → nonblocking)
matching the protocol's existing definition.

Add explicit before-commit necessity to the Evidence placeholder for
blockers, matching protocol criterion 3. Qualify the blocking impact
hint to require substantiated evidence AND demonstrated before-commit
necessity. Add canonical disposition guidance: discard unsupported or
already-handled claims; downgrade to nonblocking only if a validated,
clearly useful concern remains.

A. bugfix/pr.md Step 7: stop and report on staging mismatch instead of
auto-unstaging user work. Pre-commit verification compares actual
staged entry identities (kind+mode+payload), not just stat/status.
B. bugfix/pr.md snapshot: use git hash-object -t blob consistently for
regular files regardless of tracked/untracked origin (stage-only
transition yields same identity). Qualify mode detection: git diff
shows prospective mode for unstaged chmod; ls-files -s shows index
mode only. Normalize identity independent of M/A/R labels.
C. implement/publish.md: move revalidation check BEFORE git add/commit
(aligned with e2e's existing correct pattern). Stop if checks fail
or produce unreviewed changes.
D. ui-implement/publish.md: match actual validate report format
(## Result section with standalone PASS, ## Validated HEAD section).
Use Step 1's existing parse contract.
Assisted-by: AI assistant
implement/publish.md: make PASS-without-fixes proceed directly to
Step 3; PASS-with-fixes completes revalidation and commit first.
ui-implement/publish.md: same routing; reference actual validate
report format (## Result section with standalone PASS token,
## Validated HEAD section) matching Step 1's existing parse contract.

Suggestion: Use MINOR bumps for behavioral changes
AGENTS.md:130–133 reserves PATCH bumps for changes without behavioral impact. This PR adds candidate-convergence/staging rules and post-review validation, so these packages warrant MINOR bumps:
Agreed: the candidate-convergence/staging rules and post-review validation are direct behavioral workflow changes, not just shared-file cascades. The versions now use MINOR bumps relative to main:

ui-implement/SKILL.md: 0.3.0
This follow-up changes only those three version declarations; the cascade-only PATCH bumps remain unchanged.
Version/cascade validation and the applicable local checks pass. All GitHub Actions checks also pass on the published commit.
