file_lock: optional appender that follows an atomically replaced file #165

Follow-up to #163 (PR #164), the optional second piece: an appender that keeps its handle and reopens when its inode no longer matches the path (open per append on Windows). Starter: _LockedAppender in xdol/access.py. Should build on dol.FileLock; xdol would then import it.
