- e2e_gateway case 14: keep only the client-side X-Mq-Cache reject; the cache HIT/no-store/bypass branch and the origin's /__count counter never ran against the Rust server.
- e2e_mitm_h2: run cases g and h by default; they were opt-in only because the C binary failed them. Delete the C/Rust interop wrapper.
- Stop passing the ignored --cache-max-bytes to the test servers.
- generate-mitm.sh: regenerate a CA key that is not PKCS#8 instead of keeping a stale one the Rust loader rejects.
