Bump first-draft-axi to 0.6.0 so the release workflow publishes the polish commands merged in #105, and document polish and polish-feedback in the README and the agent skill.

first-draft-axi0.5.0 -> 0.6.0 (package.json, package-lock.json) so the release workflow publishes the polish loop merged in #105 (PR 105 did not bump, so nothing was published).
