### Fixed
- The page not found response is JSON for every client that asks for JSON, not only when the `Accept` header is exactly `application/json`: clients such as axios (`application/json, text/plain, */*`), a `charset` parameter, `+json` types and AJAX requests used to get the HTML page. `page_not_found_response()` decides with `Request::expectsJson()` and takes an optional request (#581)
- `core:env` now looks for the existing `.env` in the application base directory, so running it from another working directory asks for the confirmation instead of silently overwriting the file (#575)
- The file rate limiter creates the missing parent folders of its `path` (for example `cache/data` when `cache/` does not exist), instead of failing every request that goes through the rate limit middleware with `mkdir(): No such file or directory` (#585)

Treat a directory created by another request as success.

The web request path can construct FileRateLimitAdapter concurrently. If two requests find path absent, both can pass isDirectory(). The second mkdir($this->path, 0777, true) can then return false and emit an E_WARNING after the first request creates the directory. The web error handler converts enabled warnings into ErrorException, so the second request can fail even though the directory exists.

Handle the mkdir() result with a postcondition check. Treat an existing directory as success, and report failure only when the path still is not a directory. Suppress or capture the expected warning so the race does not become an exception.

Review comment at @src/RateLimit/Adapters/FileRateLimitAdapter.php at line 46:
Update the directory creation flow in FileRateLimitAdapter to handle concurrent
creation: suppress or capture the expected mkdir warning, then treat an existing
directory as success and report failure only if the path is still not a
directory.
